Fairly straightforward; how does one address the leader (if you have several 'leaders,' head of state will do) of your nation? I've listed a variety of situations where different terms of address may be required.

In conversation: Your Majesty, Your Royal Majesty
In writing (from a government official acquainted with your leader): To my Noble Liege of whom I address
In conversation (by an official): Sire, My King, My Lord, My Liege, Noble Sir, Dragonheart
Name and usual title: His Most Royal Majesty
Full formal title: His Most Royal Majesty, King Aubrey the Dragonheart, King of all the realm of Avaerilon, Lord of the Citadel of Minas Tirith and Yrl of Ilnys Avalon, King of Fereldan, King of Arnalor, King of Dunaemair, King of Morrowind, King of Cornlernor, King of Fenoldan, Marquis of Grand Merinale, King of Laonamea, Archduke of Luxembourg and of Belgium, Viscount of Majmehra, Earl of Merrioch, King of Montevittora, King of Nilvana, King of Orcat, Grand Captain of Pan-Goi-Nang, King of Pendouras, King of the Estorles, Grand Master of Valertha, Count of Vermora, King of Woldesia, King of Yerrisat, Nerevarine, Hortator, Dragonborn,
